As an oral surgeon, I am pleased to provide you with this guide on sinus lift procedures, a necessary surgical intervention facilitating dental implant placement in the upper jaw, particularly at the back near the sinus cavities. This information aims to assist you in understanding the procedure, evaluating the benefits and risks, and preparing for informed consent.

Understanding Sinus Lift Procedures

A sinus lift is a surgical technique used to increase the amount of bone in the upper jaw, beneath the sinus cavities adjacent to your nose, to allow for the secure placement of dental implants. This is necessary for patients who have experienced bone loss in the upper jaw or whose sinus cavities are too close to the jaw for implant placement.

The procedure involves “lifting” the sinus membrane to insert bone graft material, which integrates over time, providing a sturdy foundation for the implants. Depending on the individual case, sinus lifts can be performed simultaneously with implant placement or as a separate preliminary step.

Procedure Variants

The sinus lift technique varies based on the existing bone volume:

External Sinus Lift (Lateral Approach)

Without Existing Bone: Bone graft material is placed through an opening made on the cheek side of the sinus after lifting the sinus membrane. Implants are typically placed in a subsequent procedure after healing.

With Some Existing Bone: Similar to the above but may allow for immediate implant placement depending on the bone stability.

Internal Sinus Lift (Crestal Approach)

This less invasive method is performed through the dental implant site. A small amount of bone is tapped upwards, and the implant may be placed concurrently if only a slight bone increment is needed.

Bone Graft Materials

Various grafting materials can be used, including autografts (from your own body), allografts (from another human donor), xenografts (from animals, commonly bovine), and synthetic materials. Common choices include Bio-Oss (bovine-derived) for the graft and Bio-Gide (porcine-derived) for the membrane cover, which facilitates new bone growth and healing.

Risks and Complications

While sinus lift procedures are generally safe, potential risks include:

  • Membrane tear or puncture, which might require a patch or delay in procedure
  • Post-operative swelling, bruising, and pain, typically manageable with over-the-counter painkillers
  • Rare complications like chronic sinusitis, infection, or prolonged numbness
  • Graft failure or inadequate bone formation, particularly in smokers or those with certain health conditions

Benefits of a Sinus Lift

Undergoing a sinus lift can significantly enhance the feasibility and success of dental implants in the upper jaw by:

  • Providing sufficient bone height and stability
  • Reducing the risk of implant failure due to insufficient bone
  • Improving aesthetic and functional outcomes of dental restorations

Alternatives to Sinus Lift

Alternatives may be considered if a sinus lift is not feasible, including:

  • Shorter implants that do not require sinus augmentation
  • Placement of fewer implants with potentially different restoration methods like bridges or dentures
  • Use of angled implants to avoid sinus lifting
  • Non-implant options like conventional bridges or removable dentures

Preparing for the Procedure

Patients are advised to:

  • Maintain normal eating and drinking habits prior to the procedure
  • Continue regular medications unless advised otherwise
  • Use painkillers like ibuprofen for comfort
  • Attend the procedure on time, as most sinus lifts are performed under local anesthesia, with sedation or general anesthesia used as needed for comfort or multiple procedures.

Post-Operative Care

Following a sinus lift, you can expect a follow-up to remove stitches and monitor healing. It’s important not to blow your nose for at least two weeks and use prescribed medications to support healing and prevent complications.

Informed Consent

Before proceeding with a sinus lift, we will discuss all aspects of the procedure, risks, benefits, and alternatives with you. Your informed consent is essential for us to move forward, ensuring that you fully understand and agree to the treatment plan.
